MFA ADDENDUM

Effective as of January 1, 2024 (Version Number: 45292)

All terms not defined herein shall have the meaning ascribed to them in the DAS Standard Incorporated Terms and Conditions.

Multi-Factor Authentication– If the executed COF or Cybersecurity Bundled Package includes “MFA APP” or “MFA TOKEN” in any form, DAS will use reasonable commercial efforts to provide: 

1. Subject to and conditioned on Client’s payment of Fees and full compliance with all other terms and conditions of this Agreement, DAS grants to Client a non-exclusive, non-sublicensable, non-transferable license to access and use HPSS and tokens using the DUO Mobile Software. 
2. It is understood and agreed by the Client that Duo Security Services are provided for commercial use only, not for private use. 
3. Payment. DUO services are Services Billed in Arrears and subsequent MSC adjusted accordingly. 
4. The DUO End User License Agreement (EULA) is available at: https://dashealth.com/documents and is specifically incorporated by reference and made a part of this Agreement. 
5. MFA tokens may be purchased on behalf of the Client if a COF is executed, and a UFC is paid.  Tokens may be transferrable but do not have a warranty and new tokens will need to be ordered if replacements or additional inventory is needed. 
6. The MFA Azure AD token will be configured with the Client’s existing subscription of Azure AD and does not include a subscription for Azure AD.
7. The MFA DUO token will be configured with the Client’s existing subscription of DUO and does not include a subscription for DUO.
