Easy Setup and Major Benefits

With easy-to-use technology, your patients automatically send you real-time monitored data. Our device technology is run on the AT&T network, meaning you don’t need to worry about internet outages or Wi-Fi setup. Our Care Team will identify eligible patients, complete accelerated enrollment, provide the appropriate equipment, assist with initial setup, and educate the patient on the benefits of the program.

  • Remote Patient Monitoring is available for patients with an acute or chronic condition that would benefit from physiologic monitoring.
  • RPM must provide at least 20 minutes of clinical interaction per month, per patient and requires patients to monitor their vital signs for at least 16 days out of the month.
  • Patients must be established with the practice, having been seen within the last 12 months.
